PTSO Membership Form

Please complete a membership form for each person that is joining the NHS PTSO. This will give us accurate information about all the fun activities we have planned for the year. Membership is $5 per person and your membership isn't complete until payment is received. Please submit your payment by turning in cash or check to the NHS Front Office.
